If you are a member of the Williamstown Cannons Basketball Club (which is an affiliated club with an official Basketball Association) and you have suffered an injury whilst participating in an official activity (including training, games, competitions or other sanctioned activities) please follow the below steps to lodge a Williamstown Cannons Basketball Club “Incident / Injury Report Form” and if required a Personal Accident claim with the Basketball Victoria Insurance company.
Please refer to the Policy Wording for policy terms, conditions and exclusions and note that only PAID (Financial) members for the current season are covered by insurance.
The injury should be documented on the scoresheet and also a Williamstown Cannons Basketball Club “Incident / Injury Report Form” must be completed including the nature of the injury (ie leg, arm, head), date and time it occurred and first aid/ambulance assistance.
This is particularly important when injuries occur at training or any other club sanctioned function as there is no scoresheet to record injuries.
Any supporting photos, notes or witness details should also be noted where relevant.
Send the Williamstown Cannons Basketball Club “Incident / Injury Report Form” to the Club Secretary at secretary@williamstowncannons.org.au.
Seek medical attention as soon as possible to determine the nature of the injury and treatment required. Advise your Coach and Team Manager if you are unable to attend future trainings or games or require a modified training/playing schedule.
